We’re honored you’re considering us for your care. Here’s what you need to know before your first visit so check-in is simple and stress-free.
Your first visit: what to expect.
When you arrive, our front desk team will check your information, verify your insurance, and help you complete any remaining forms. You’ll then meet with our nursing staff and your provider to review your health history, current concerns, and goals for your visit.
Most new patient visits include time for questions and education about your health, not just a physical exam. We encourage you to write down any questions ahead of time and bring them with you.
If you’ve recently had labs, imaging, or gynecologic care elsewhere, let us know so we can help request those records before your appointment whenever possible.
What to bring to your appointment.
- Photo ID (driver’s license or other government-issued ID).
- Your current insurance card(s).
- Any referral required by your insurance plan.
- A list of current medications, vitamins, and supplements.
- Information about allergies and past surgeries or hospitalizations.
- Copies of recent lab work, imaging, or records if you have them.
- Completed new patient forms (if you filled them out ahead of time).
Please arrive 15–20 minutes early to allow time for paperwork and check-in—especially if this is your first visit or your information has changed.

Appointments & cancellations
- Please call as soon as possible if you need to change or cancel your appointment.
- A minimum advance notice (for example 24 hours) helps us offer that time to another patient in need.
- Repeated late cancellations or missed appointments may result in a fee, per office policy.
- If you are running late, call the office—our team will let you know if we can still see you that day.
Insurance & payment information
- We participate with many major insurance plans; coverage depends on your specific policy.
- Co-pays are due at the time of service, as required by your plan.
- Deductibles and co-insurance may apply based on your benefits.
- If you do not have insurance, ask about our self-pay options at the time of scheduling.

Patient portal & questions
- Use our secure patient portal to send non-urgent messages, request refills, and view some test results.
- Please allow time for our clinical staff to review your message and respond during office hours.
- For urgent concerns, abnormal bleeding, severe pain, or questions after a procedure, call the office directly.
- In a medical emergency, always call 911 or go to the nearest emergency room.
